Journal Club 6/15/23 - Real-world Use of Bezlotoxumab and Fecal Microbiota Transplantation for the Treatment of C. diff Infection

On Thursday, June 15th at our monthly Infectious Diseases Journal Club, Dr. Swetha Srialluri led discussion on this study looking at real-world use of bezlotoxumab and fecal microbiota transplantation for treatment of Clostridioides difficile infection.

 

We compared the results of this study to those from RCTs for bezlotoxumab and oral FMT, and discussed the roles of each of these therapies in management of CDI patients at high-risk for recurrences and those who have had multiple recurrences. We also discussed cost considerations and implementation of these interventions.

Our second-year fellow, Dr. Brianna Hatch-Vallier, traveled to Nepal with PD, Dr. Holly Murphy, in November 2023 on the inaugural global ID elective the fellowship is now offering. This elective offered a chance for Brianna to spend time at the Patan Academy of Health Sciences (PAHS). PAHS hosts the first ID fellowship in Nepal and one of the first in the region. PAHS is a national TB and referral center with a medical school, residences, and a long history of hosting and teaching international doctors-in-training.  Brianna gave a lecture on 12/21/23 to Trinity Health's faculty recapping interesting cases from Nepal and reporting on her overall experience. We are very grateful for this connection that Dr. Murphy has fostered, as it allows future fellows this unique learning experience of global ID.

The first to answer and the first to answer correctly will be rewarded with a treat.   A 51 yo man was seen in the emergency room in New Orleans for a headache for one day.   He lives on a farm outside of the city and eats raw goat’s milk. Two days prior to presentation he drove his truck into a lake and ingested a fair amount of water and had an injury to his leg as he tried to pry open the door and get out of the vehicle. He denied trauma to the head. He was awake, alert and oriented but had a stiff neck.  The rest of the exam was unremarkable.   CSF showed the following:   WBC 1672 (85% neutrophils) RBC 50 Glucose 22 Protein 480 Gram stain negative   He was treated with dexamethasone, vancomycin and ceftriaxone for suspected bacterial meningitis.  The following day he worsened with confusion, vomiting.  Cultures of CSF were negative at 72 hours.   What is the most likely etiology?   A.      Acanthamoeba castellanii B.
