In Journal Club we covered " Opportunistic Treatment of Hepatitis C Infection Among Hospitalized People Who Inject Drugs (OPPORTUNI-C): A Stepped Wedge Cluster Randomized Trial" by Midgard, et al that was published in CID March 2024. This paper evaluated the effect of initiation of DAA for treatment of hepatitis C upon discharge from the hospital as compared with standard of care (outpatient referral to specialist). Our discussion focused on interpretation of the findings, applicability and feasibility of applying the studied method into our practice.

Special Lecture - Global ID Elective in Nepal

Our second-year fellow, Dr. Brianna Hatch-Vallier, traveled to Nepal with PD, Dr. Holly Murphy, in November 2023 on the inaugural global ID elective the fellowship is now offering. This elective offered a chance for Brianna to spend time at the Patan Academy of Health Sciences (PAHS). PAHS hosts the first ID fellowship in Nepal and one of the first in the region. PAHS is a national TB and referral center with a medical school, residences, and a long history of hosting and teaching international doctors-in-training.  Brianna gave a lecture on 12/21/23 to Trinity Health's faculty recapping interesting cases from Nepal and reporting on her overall experience. We are very grateful for this connection that Dr. Murphy has fostered, as it allows future fellows this unique learning experience of global ID.

ID Board Review Question

The first to answer and the first to answer correctly will be rewarded with a treat.   A 51 yo man was seen in the emergency room in New Orleans for a headache for one day.   He lives on a farm outside of the city and eats raw goat’s milk. Two days prior to presentation he drove his truck into a lake and ingested a fair amount of water and had an injury to his leg as he tried to pry open the door and get out of the vehicle. He denied trauma to the head. He was awake, alert and oriented but had a stiff neck.  The rest of the exam was unremarkable.   CSF showed the following:   WBC 1672 (85% neutrophils) RBC 50 Glucose 22 Protein 480 Gram stain negative   He was treated with dexamethasone, vancomycin and ceftriaxone for suspected bacterial meningitis.  The following day he worsened with confusion, vomiting.  Cultures of CSF were negative at 72 hours.   What is the most likely etiology?   A.      Acanthamoeba castellanii B.
